Student Disability Resources

At Penn State, we are committed to providing a welcoming, encouraging, and empowering environment for students with disabilities to ensure equal access, full participation, and reasonable accommodations for their academic pursuits.​

Student Disability Resources (SDR) is responsible for coordinating support services, reasonable academic accommodations, and promoting disability awareness in the university community.​

Steps required to apply for services offered by Student Disability Resources​

Students who are seeking services at Penn State must self-disclose the need for academic accommodations, auxiliary aids, and/or services to the Student Disability Resources office at the Penn State campus they are attending.​

The student must demonstrate through appropriate documentation that their disorder or impairment meets the definition of a disability under Title II of the ADA Amendments Act (ADAAA) of 2088 and Section 504 of the Rehabilitation Act of 1973. A disability is defined as a physical or mental impairment that substantially limits one or more major life activities.

Application Steps​

PART 1​

  • Complete the online Introductory Questionnaire: (login required) (Note: If you receive a proxy error or internal server error message when accessing the form, please use a private/incognito web browser to resolve the issue.)​

PART 2​

  • ​Submit documentation on our secure portal. Visit for more information. Alert! Student Disability Resources does not recommend emailing documentation as email is not a secure method of sharing confidential information.​
